How to look up an NPI by name
To find an NPI by name, search the CMS NPPES registry with the provider’s last name plus a state, adding a first name or city only if there are too many results. Names must match the registry spelling, and a trailing asterisk works as a wildcard, for example “smi*” for Smith or Smithson.
Step by step
- Open the NPI lookup form.
- Enter the last name and choose the state where the provider practices.
- Add a first name or city only if the list is too long.
- Open the matching result and compare the specialty and practice location.
Search tips that work
| Situation | Try this |
|---|---|
| Unsure of the spelling | Use a wildcard: first two or more letters plus an asterisk (“joh*”). |
| Very common name | Add a state and city, or a first name. |
| Looking for a clinic or hospital | Use the organization name field instead of a person’s name. |
| No results | Remove fields one at a time. Registry names can differ from names in common use. |
Individuals versus organizations
Individual providers have Type 1 NPIs and organizations have Type 2 NPIs. A solo practitioner who is incorporated may have both. Read NPI types and taxonomy codes for the difference.
Check before you rely on a match
Several providers can share a name. Confirm the specialty, the practice address and the “last updated” date. For licensure, check the state licensing board directly.
Frequently asked questions
Can I search by phone number or address?
The NPPES search supports city, state and postal code, but not phone number. Searching a phone number requires the provider’s own listings.
Why does a provider show more than one address?
The registry stores a practice location and a mailing address, and some providers list additional practice locations.
